Features of Tuberous Breast
- Narrow breast (footprint)
- High breast fold (IMF)
- Deficient volume lower half of breast
- Deficient skin lower half of breast
- Large, puffy nipple (herniated)
- Asymmetry between sides (sometimes only one side is affected or one side is more severe)
Tuberous Breast Causes
You were born this way! Tuberous breast occurs when there are tight bands present in the breast during development. These constricting bands prevent the lower half of the breast from developing normally and can lead to any range or severity of the characteristics described above. There is a wide spectrum of cases. Mild cases may only lack volume in the middle, bottom part of the breast (lower medial pole). Severe cases appear completely tubular with all of the features described above
Tuberous Breast Treatment
Just as there are a wide range of types of tuberous breast, there are a wide range of treatments for tuberous breast. Treating tuberous breast with an implant without correcting the underlying features can lead to an unpleasing result.
Example of a patient who received breast implants without addressing the unique features of tuberous breast at another facility.
First Stage Reconstruction
Removal of submuscular breast implants. Creation of new, subglandular pocket and release of constricting bands.. Placement of a smooth tear-drop shaped tissue expander in the subglandular plane to preferentially expand the deficient skin in the lower half of the breast. The breast fold was lowered with the help of an internal bra
Next steps will include filling of the tissue expander over the course of the next few weeks. Once the appropriate space has been created in the lower half of the breast, the tissue expander will be replaced with a permanent implant. A reduction in nipple areola complex size and small peri-areolar or donut shaped reduction will be also be performed at this stage.
What can be done to treat it?
Treatment depends on the severity of the tuberous breast and may involve
- One stage treatment with release of bands and placement of a breast implant
- Reduction of nipple areolar complex size
- Donut-style, or periareolar breast lift
- Two stage treatment with tissue expander at first stage and exchange for permanent implant at second stage
- Combination of the above
