Our Approach to Mommy Makeover in NYC
It is an exciting time for Mommy Makeover surgery in NYC. We are entering an era of surgery that is truly defined by mastery. Beyond the technical wizardry in mommy makeover surgery that is happening we are experiencing a hospitality revolution at Neinstein Plastic Surgery.
From the moment I read Danny Meyer’s book “Setting the Table” I realized that hospitality is not something that only belongs in restaurants, I realized that hospitality was the missing puzzle piece in medicine, and I would devote my life to integrating hospitality into our plastic surgery practice. We as a team talk about the experience and hospitality, we provide for our patients all the time. We want it to be a living breathing way of life for our staff. While we have certain things we do such as the daily house calls by our surgeons and the text group with the patient and nurses having hospitality as a way of life means spur of the moment acts of generosity and kindness. Just this week, as I am writing this, one of our patients from out of town who was kosher was feeling left out of the sabbath back at home and our head of operations, Mitch, went out and not only got a kosher meal but bought sabbath candles and went and had a sabbath meal and ceremony with the patient. These are the kinds of things our teammates do. There is no script or playbook, and they don’t seek admiration or recognition (even though I do) because they have completely bought into the concept of hospitality at all times for all patients.
We have the incredible honor and privilege of performing 1000 mommy makeovers a year in NYC at Neinstein Plastic Surgery. This is not just a privilege but a huge responsibility. When we start our surgical days, we take a brief pause to remind ourselves that we are not operating on a patient. We are operating on a mother, a wife, a sister. We pause and reflect to remind ourselves of how our actions will impact forever a large group of people. That little pause is really important because it re-orients everyone to the intensity of the moment. The moms and the families are giving us their complete trust for keystone of their family. We will not just perform the best operation possible, but we will provide an experience that I truly believe cannot be found anywhere else. People don’t just remember the surgical results they remember how each team member made them feel at every touch point. That’s why every touch point is an opportunity to make an awesome memory.
Those first few days after Mommy Makeover surgery are crucial days for multiple reasons. We do big surgery (almost always breast and body together because people want to get it all done in one session), which means the safety of the mommy makeover patient in those first few days is paramount. I have created an incredibly comprehensive aftercare program for our mommy makeover patients that involves 24/7 private nursing and daily house calls by your plastic surgeon. The private nurses have been trained to monitor blood supply, analyze swelling, bleeding, and bruising. They have a strict protocol for taking vital signs and giving medications. All this information in real time is being shared with the surgeons and the nurses. This system reduces the stress and anxiety of not just the patient but the spouse and family who are not physically present. I can’t imagine doing the operations we do (mommy makeover) and sending the patients home alone to fend for themselves. These first few days are not just about safety they are about being there to emotionally support the patient who is recovering from her mommy makeover and to make a beautiful and memorable experience. A brief aside, all of this is only possible if you have a team of a-plus team members who are all in. I spend a lot of time not just recruiting, training, and retaining talent but ensuring they feel loved and valued. Patients are keen observers. It’s important that they see that all the team members want to be there, that all the team members are fully devoted and passionate, and the team members are happy and working with a sense of joy.
Because social media allows us to showcase our efforts to the world, we can pick one operation for instance mommy makeover for me and devote our entire practice to it. Because social media can share it with the world, we are able to attract that many patients to have a whole practice based on one operation. This was not really possible before because patients did not travel for surgery like they do now so the surgeons in your area were forced to do everything.
For the fourth time I recently read Robert Greene’s book on mastery, and this is how he distills down what mastery means:
“We enter a childlike state of curiosity, we access a mental dimension of creativity and spontaneity that only children witnessing the world for the first time appreciate. Masters can create new because they literally are able to enter a dimension of thinking about the problems that the rest of the world simply cannot understand because they don’t have the thousands of hours of programmed neural circuitry. This latent power is in every human, yet few will ever devote the amount of time to a single task to ever get there. The practical power is we unlock the possibilities of a new reality for our chosen task”
Mommy Makeover Surgery: My Life’s Work.
This is how I approach mommy makeover surgery. It is my life’s work.
Big Picture for someone new to Mommy Makeover plastic surgery – and especially someone contemplating Mommy Makeover surgery in NYC:
- We almost always combine breast and abdominal surgery at the same time for mommy makeover surgery at Neinstein Plastic Surgery. This is because this is all we do. After literally performing 1000’s our surgeons, anesthesiologists, and surgical team can safely and predictably perform the most complex of surgeries in under 6 hours. 6 hours is the “golden rule” for safety in Mommy Makeover surgery. For instance, here is an example of what can now safely and predictably be done in under 6 hours of mommy makeover surgery.
Breast Surgery: breast lift with breast implants along with the use of an internal bra and fat grafting for custom cleavage.Abdomen: we can perform lipo360, full tuck-extended tuck- circumferential tuck with complex diastasis recti repair.Doing the breast and body together in one surgical session should only be done by masters of mommy makeover surgery. I say this for several reasons. First and foremost is safety but second is harmony. To create harmony in the torso with so many moving parts you really want not just a surgeon but a surgical team that performs these mommy makeover operations every single day.
- The diastasis recti repair has completely evolved. In the last year we have completely moved away from the traditional up and down in the midline rectus repair alone being the abdominal wall repair. We kept finding that moms were coming back with bulges. We put our heads together and spoke to everyone we could around the world and realized that multi vector meaning repairing more than just the up and down separated muscles had to be done routinely and not just in special mommy makeover cases we were seeing during mommy makeover operations. This means we are routinely doing custom muscle repairs during tummy tucks. Again, this is giving unprecedented improvement in mommy makeover surgery results. We are doing transverse and oblique repairs, and we are simply just repairing everything that bulges. We were all taught to only do the up and down muscle repair with the tummy tuck and to leave it at that. But this was not good enough. The moms were not getting flat enough tummies after their mommy makeover surgery so we had to evolve. This again needs to be done by an expert because you can over tighten the muscles. This can cause permanent issues to the abdominal wall. There is a lot of knowledge for the kinetics and physiology of muscle function that the surgeon must understand. A lot of this knowledge cannot be found in textbooks because we are writing these textbook chapters now but we do have the knowledge and we are repairing muscles in a better way during mommy makeover surgery than we ever have
A few things to know about multi vector muscle repairs during Mommy Makeover surgery:
- The patient must receive Exparel which is a long-acting numbing or pain medication that gets injected into the muscles during the diastasis recti part of the operation.
- I do not believe it is safe to do multi vector diastasis recti muscle repairs and send the patients home. These mommy makeover patients are required at Neinstein Plastic Surgery to be monitored overnight by our private nurses who have specific training in our monitoring protocols.
- The surgeon must see and evaluate the abdomen in the morning after surgery. If you are going to have a mommy makeover in NYC I highly recommend ensuring you are being seen early in the morning after your surgery by your surgeon.
A few things on the length of the scar in Mommy Makeover Surgery
Most people think tummy tuck surgery was to make big people into small people but our thoughts and understanding of tummy tuck and Mommy Makeover surgery has evolved its really to make people tight. The old way of thinking about the length of the incision was to break it down into mini, full, extended, and circumferential. Almost like there was a menu and people got to choose. Its very important to understand that you don’t get to pick the length of the incision you only get to pick if you win or lose at your tummy tuck.
What do I mean by you don’t get to pick the length of the scar?
Your anatomy will dictate the length of the scar. I have learned that to provide harmony we have to remove as much loose and extra skin there is. It just looks weird when you leave loose skin. I mean who would want to be left with loose skin. 50% of my practice is revision tummy tuck surgery and its almost always that the scar was not long enough. So, nothing bad happened during their original surgery but nothing great happened either and the patient wanted greatness
Why are our scars longer for tummy tucks now than they used to be.
- GLP-1 medications. Ozempic, Mounjaro etc. have created more loose skin than we have ever seen before. See our deep dive into Ozempic and plastic surgery. So we now have a new problem- more loose skin. We can’t use old solutions for new problems. The legacy or traditional tummy tuck which most surgeons still use is only designed to operate on the front of the body so that means the loose skin on the waist and back will not be addressed. People don’t want to just be narrow and loose they want to be narrow and tight. The benefit of having four surgeons working together is the brain power. The four of us got together and literally re-designed the modern tummy tuck. We figured out how to apply our long, lean, and elegant operating philosophy to extended and circumferential tummy tucks. The moms who wear string bikinis have extended or circumferential tummy tucks because a) the scar fades and b) they have their problems completely solved.
- The results that were considered by patients good in 1994, 2004, and 2024 are no longer considered good. Patients’ desires have evolved. They want more than just some skin removed. If they are going to take the risk of having surgery and have their lives completely disrupted by the downtime and cost, they want more out of it. They really want to do it once and do it right and get transformed.
In your consultation our doctors will draw and show you exactly how long the scar should be to get the most long lean and elegant results for your body.
New scar and pain management therapies for mommy makeover surgery in 2025
Sylke
Groundbreaking re-invention of one of nature’s premiere biomaterials-silk. This company ethically sources its silk from Norther Italy. This has replaced steri- strips. It’s simply a much better dressing on the horizontal part of breast lifts and around the areola. It holds things together to get a better scar for two weeks after surgery.
Exparel
Game-changer for pain control in the abdomen. Most numbing medications last 12 hours, Exparel lasts three days. We inject it into the abdominal wall after the muscle repair. This is a game changer because it allows patients to get up and get moving.
Brijjit
This tissue modulation system is applied to the vertical component of the breast lift scar to enhance scar healing.
Prevena Incision Management System
Another game-changer. This is a device we place over the tummy tuck scar at the end of the operation. It goes on the skin not in the skin. It causes a lot of blood flow to the tummy tuck scar. More blood flow means a better scar and less infections or wound healing problems
Ok the next part is for the nerds and it’s also a way for me to share insight into how I think. While you are on your journey of choosing a surgeon for your Mommy Makeover it’s important to get to know your surgeon because you are placing a lot of trust in this person.
The Process of Mastery in Surgery
What is the process of mastery in surgery?I took a lot of this from Robert Greene’s amazing book Mastery.
- In the beginning we are outsiders- when we first start, we are intimidated, and we don’t understand all the relationships that contribute to success.
- We never give in to the difficult process of learning, as we continue to observe and learn from others, we start to gain clarity.
- As we continue with our hyperfocus on a set of skills like those required to perform a mommy makeover we start to gain fluency.
- We start to see connections and maneuvers that were invisible before because we didn’t have the neural circuitry required to process such a vast amount of information all happening at the same time as in what is happening during a mommy makeover in NYC.
- Through shear persistence we use our expanding knowledge to get increasingly creative and more confident. This is the stage most surgeons will never get to for an operation. The reason most will never get too is because you must consciously decide to abandon everything else in your practice. It’s very hard to say “ok that’s it no more botox, filler, laser, face, eye, nose surgery, that’s all done. I am going all in on this one thing”. The reality is that it’s scary economically to say NO to business for a surgeon. But i believe behind every no is a more meaningful yes to the patients.
- We learn from the process of deep work (that is detailed evaluation of our results) what is safe and where the edges of safety are.
- As years go by another leap takes place- a leap to mastery. We start entering a creative state. We can rewrite what is possible.
- We enter a childlike state of curiosity, we access a mental dimension of creativity and spontaneity that only children witnessing the world for the first time appreciate. Masters can create new because they literally are able to enter a dimension of thinking about the problems that the rest of the world simply cannot understand because they don’t have the thousands of hours of programmed neural circuitry. This latent power is in every human just so few will ever devote the amount of time to a single task to ever get there. The practical power is we unlock the possibilities of reality for our chosen task.
This is a fully immersive process for a surgeon – it is a lifetime of work.
